Fidji Simo, the second-ranking executive at OpenAI, is stepping down from her role as head of product and business after a severe flare-up of a chronic health condition forced her to reassess her position at the artificial intelligence company.
Simo disclosed Thursday that she experienced a significant worsening of Postural Orthostatic Tachycardia Syndrome, or POTS, a condition she has managed since 2019. The exacerbation prompted her medical leave in April, during which OpenAI President Greg Brockman took over her product responsibilities. She will now transition to a part-time advisory role at the company.
In a post on X, Simo reflected on the difficulty of her decision. "I am only making this decision now because I failed to make it many times before," she wrote, noting that her experience with chronic illness has sharpened her focus on AI applications in healthcare.
OpenAI CEO Sam Altman responded publicly, expressing sadness over the departure while praising Simo's contributions. "We all wish her the best for a speedy recovery. this sucks," Altman posted on the platform.
Simo's exit marks a notable shift in OpenAI's leadership structure. She had joined the company in 2025 after steering Instacart through its initial public offering and spent over a decade as an executive at Meta. Altman recruited her specifically to expand OpenAI's business beyond cutting-edge AI research into practical consumer and enterprise applications as the startup broadened its commercial strategy.
Her departure removes one of OpenAI's most visible leaders during a critical growth phase for the company.
