An outbreak of cyclosporiasis is laying bare the fragmented systems and resource constraints that undermine the nation's ability to detect and contain foodborne illnesses quickly.
The parasite has sickened more than 11,000 people across the country, sent over 300 to hospitals, and forced investigations into six separate outbreaks. The culprit identified so far: iceberg lettuce from Taylor Farms de Mexico, which triggered recalls in 42 states after Taco Bell customers fell ill in nine states. Michigan bore the brunt, with more than 7,000 cases reported.
Experts say the sheer scale of the outbreak reveals systemic vulnerabilities that have worsened with recent budget cuts. Barbara Kowalcyk, director of the Institute for Food Safety and Nutrition Security at George Washington University, called the magnitude "surprising" and described it as symptomatic of deep weaknesses in how America tracks and responds to foodborne disease.
The disease itself, caused by a microscopic parasitic protozoa, produces weeks of watery diarrhea and spreads through food and water contaminated with human feces. Summer outbreaks are common enough that public health workers have dubbed it "cyclospora season." This year's explosion, however, signals something has broken down.
The detection process begins when a sick person visits a doctor willing to test for the parasite, which doesn't appear on routine stool sample panels. That immediately undercount cases. If positive, the physician reports to the state health department, which launches an investigation to determine whether the illness was acquired domestically or abroad. Only then does the state forward the case to the CDC through the National Notifiable Diseases Surveillance System.
Not every state mandates reporting. Pennsylvania, for example, makes it voluntary. Even when required, reports lag by roughly six weeks from illness onset, creating a temporal gap that can slow response efforts. The CDC relies on these delayed reports to initiate advanced testing like genotyping to pinpoint outbreak sources.
Compounding the problem is a critical funding collapse. The Trump administration cut $11.4 billion in pandemic-era grants to state and local health departments in 2025, crippling their outbreak response capacity. The CDC itself lost 25 to 30 percent of its workforce. Within the CDC's parasitic disease unit, staffing dropped from 11 specialists to three, partly because USAID funding that supported the team was slashed.
Communication adds another layer of confusion. The CDC's official outbreak website counts only cases with both confirmed lab results and interviews. Michigan health authorities, by contrast, report all lab-confirmed cases to the CDC even when interviews remain incomplete. These different definitions explain why state and federal case counts diverge, frustrating public health officials and the public alike.
Kowalcyk underscored the consequence of this fragmented system meeting reduced resources. "Ideally, we would identify and solve these outbreaks when they're small," she said. "You don't want an outbreak of 11,000 people. It's symptomatic of the fact we have cut public health funding over the last two years to an area we haven't fully funded to begin with."
