A single dose of psilocybin produces measurable physical changes in the brain that persist for weeks, according to a study by researchers at UC San Francisco and Imperial College London published in Nature Communications. The findings offer new insight into why the psychedelic has shown promise for treating depression, anxiety, and addiction.
The research involved 28 healthy adults with no prior psychedelic use and no diagnosed mental health conditions. Participants first received a 1 mg placebo dose, then returned one month later for a 25 mg dose capable of producing a full psychedelic experience. Brain activity was monitored using electroencephalography (EEG) during the trip, with additional imaging scans performed in the weeks following each dose.
Within 60 minutes of the 25 mg dose, EEG recordings detected a sharp rise in brain entropy, a measure of the variety and unpredictability of neural activity. The finding indicates psilocybin triggers the brain to generate and process a far broader range of information than baseline. One month after the experience, diffusion tensor imaging revealed that neural tracts had become denser with greater integrity, a pattern opposite to what occurs during normal aging.
Twenty-seven of the 28 participants described the psilocybin experience as the single most unusual state of consciousness they had ever encountered. The day after the trip, these same participants reported substantially greater psychological insight compared to after the placebo dose. More importantly, those who experienced the largest increases in brain entropy during the acute effects were also most likely to report heightened insight the following day.
Measures of well-being improved at two weeks and four weeks post-experience. Participants endorsed statements like "I've been feeling optimistic about the future" and "I've been dealing with problems well" at higher rates than after the placebo. One month after the psilocybin dose, participants also performed better on cognitive flexibility tests, which measure the ability to adapt thinking in response to new information.
Senior researcher Robin Carhart-Harris, the Ralph Metzner Distinguished Professor of Neurology at UCSF, noted the significance of the findings. The psychedelic experience itself, not just the pharmacological action of the drug, appears central to its therapeutic effects. "The trip and its correlates in the brain is a key component of how psychedelic therapy works," Carhart-Harris said.
The chain of effects became clearer in the data: participants with the highest brain entropy during the acute phase reported greater insight the next day, and those with greater insight showed stronger well-being improvements one month later. This suggests the psychological breakthrough during the experience drives the longer-term mental health gains.
Research associate Taylor Lyons of Imperial College London, the paper's lead author, emphasized the broader implications. "Psilocybin seems to loosen up stereotyped patterns of brain activity and give people the ability to revise entrenched patterns of thought," Lyons said. "The fact that these changes track with insight and improved well-being is especially exciting."
The study's use of healthy participants without diagnosed mental illness allowed researchers to conduct a wider array of neuroimaging tests than would be possible in clinical patient populations. This breadth may help explain the mechanisms behind psilocybin's therapeutic potential without the confounding factors present in people with active psychiatric conditions.
Researchers cautioned that additional work is needed to clarify what the structural brain changes observed one month later actually signify functionally. Still, they described the finding as the first evidence that a single psychedelic dose produces lasting structural changes in the brain.
The results could eventually guide refinement of psilocybin therapies for mental health conditions. One practical application would be identifying doses that produce sufficient brain entropy to catalyze meaningful psychological insight without triggering an unnecessarily overwhelming experience. As Carhart-Harris noted, "We already knew psilocybin could be helpful for treating mental illness. But now we have a much better understanding of how."
